J. Summary of student's performance and commitment and any other key information
A) Commitment and performance indication schedule
B) How to interpret above schedule;
Example
For Physics Scholarship recipient has obtained 40 marks in
the school term test held on December. For the respective term (September,
October, November) his AS % (Assignment Schedule) for 80%-100% range is 55% .
Also his marks and AS % for the same subject for
April term test is 60 and 70% respectively.
What this means
Scholarship recipient has positive movement in relation to AS
% (from 55% to 70%), this means he has effectively completed assignments during
the April term (December, January, February and March) compare to previous December
term.
C) Who shall prepare schedule;
This shall prepare and update by the Chair based on the information received from Supervisors.

I. Corrective procedures (Form F)
A) Corrective procedures summary

C) How to complete
Supervisor shall complete form F in
which lists the reasons for the Scholarship recipient referring to the Four (4) stages Corrective procedure.
Step 1 – Directs to the class
teacher with their parent together with the Supervisor and get the class
teacher’s advice and guidance on corrective measures required.
Agree on the corrective measures
that Scholarship recipient is required to take.
Scholarship recipient duly complete
all corrective measures to the satisfaction of the Supervisor.
Step 2 – If the Scholarship
recipient doesn’t duly complete any of above corrective measures to the
satisfaction of the Supervisor, Scholarship recipient directs to the head of
the respective A/L section for further advice and guidance together with their
parent(s).
Step 3 – If the Scholarship recipient
doesn’t duly complete any of above corrective measures to the satisfaction of
the Supervisor, Scholarship recipient directs to the Principle further advice
and guidance together with their parent(s).
Step 3 – Scholarship committee may
suspend/terminate your scholarship.
D) Reasons to refer a student to corrective procedure
1) Scholarship
recipients failed to achieve 100% all the assignments listed in Form C at the
end of each term.
2) Scholarship
recipients failed to achieve stipulated minimum SHR percentage (this is to decide by the Committee) for each term.
3) If the
performance drops (i.e. main subject's marks) by -10%,

How to read above tables
Example
At the end of September Scholarship recipient has completed 11% of his assignment more than 80% but at the end of October he has completed 15% of his assignment more than 80%.
What it says
It seems to be the Scholarship recipient has not commitment to complete his assignments within the respective month (viz. not working on time) but has positive movement for the same (viz. positive movement for commitment to study on time).

How to read above tables
E.g Physics/2017/Oct;
During the month of October the Scholarship recipient has completed 15% of his assignments more than 80%. Subsequently he has completed 40% of his assignments more than 80% during the month of November.Finally he has completed all their assignments 100% at the end of December.
What is says
It seems to be the Scholarship recipient has not commitment to complete his assignments within the respective month (viz. not working on time) but he has commitment to complete assignments, as he has achieved 100% in the month of December.
Scholarship recipient
Step 1- There should be one From C for each subject for each
month. Scholarship recipient shall list all assignment (including homework)
(from both tuition class and school) in the Form under t2nd column.
Step 2 - Scholarship recipient shall states the completed
percentage of each and every assignment under 3rd column.
Step 3 - Scholarship recipient shall categorise completed
assignment percentages under the ’Summary’.
E.g. During the month of September 2017, you have been given
4 assignments for Physics;
Assignment 1 you completed 100 %( viz. answered for all
questions regardless the correctness),
Assignment 2 you have completed 50% (only you have completed
20 questions out of 40),
All other assignments have completed 10% only (only you have
completed 1 page out of 10 pages).
If so, your summary for Physics for the month of September shall
as follow;
<20% = 2/4 =50%
20%-50%=0/4=0%
50%-80%=1/4=25%
80%-100%=1/4=25%
Step 4 – You shall give the completed Form C s to your Supervisor
at the end of each and every month and get his consent.
Step 5 – You shall continue to complete any incomplete
assignments. You shall state cumulative percentage against each and every
assignment in the 4th and 5th column following each
subsequent month, respectively, according to above example during the month of
October and November.
Step 6 – Update summary section also accordingly.
Step 7 - At the end of each allocated time from to complete
assignments (i.e. Three months), you shall achieve the 100% all assignments, otherwise
you may direct to a corrective procedure.
Step 8 – Any achievements following initial three months period,
shall demonstrate to the Supervisor and get his comment against the respective
assignment.
Supervisor
Step 1 - Review the completed Form C following submissions, advice
and guide the Scholarship recipient accordingly. At the end of each allocated
time from to complete assignments (i.e. Three months) you may direct them for
the corrective procedure; you may grant an appropriate extension of time to
complete From C.
Step 2 – You are required to send summary information of
Form C to the chair following the completion of allocated time from to complete
assignments (i.e. Three months).
Step 3 - Any achievements following initial three months period
also required to communicate to the Chair.
